The confidential medical dental questionnaire is kept in your dental file, and your dental file is kept in a cabinet to which the public does not have access.

Only healthcare staff will have access to your health information, administrative staff will only have access to the information necessary to carry out their duties (redirection to a healthcare professional/specialist, billing, appointment making, transmission to insurance companies).
